Planning Your Arrival to Page Middle School Please call our School Counseling Department at 804-693-2540 (ext. 1) to make an appointment to register your child. Registration Requirements: Original birth certificate Updated Immunization records Transcript from Former School (can be unofficial) with last report card IEP and/or 504 Plan (if applicable) Court Orders Referencing Student (if applicable) Proof of Residence: Contract from purchase of house/lease/rental agreement Electric Bill Upon arrival to Page Middle School, you will be asked to present a valid driver's license or identification card at the Welcome Desk.
